How to complete this form:

  • Consider the various tasks undertaken during the activity or event.  Those that have the potential risk to cause harm should be listed.  Eg. Swimming has inherent risks during a youth outing.  Accidents may occur during fishing at a church fishing camp.  This form has capacity for 10 possible tasks that may have risks.  If your activity has more tasks than this, please complete another form and indicate Part 2 in the activity name.
  • List the risks associated with these tasks.  Eg. The burning sun during a youth camp game in the middle of the day or the chance of getting a fishing hook embedded in your hand as part of a church fishing camp.
  • Determine the likelihood of a potential risk using the following table:

  • Nominate control measures to mitigate against the risks identified.  Eg. Advise players to wear hats and apply sunscreen to avoid getting burnt from the sun during a youth activity in the middle of the day.  Another example might be to ensure people are advised to keep a 2 m distance from one another during fishing activities and be advised to ensure they are particularly careful when casting lines so as to not interfere with others. Apply simple common sense measures to guard against risks.  The more complicated the risk, the more extensive the control measures need to be.  For example, swimming would require in addition to reminders about water safety the added protection of observers to ensure the safety of swimmers at all times.
  • Nominate the person responsible for applying the control measure.  Eg. the first aid officer may remind people to apply sunscreen.  A games organiser might advise on how to play a game in a safe manner.  A supervisor may be responsible for ensure safe equipment for an outdoor adventure.  Each control measure should have a name against it to ensure someone is responsible for ensuring the control measure is enacted.
